The five response options for each question are as follows: (a) never happened in the past year or at all; (b) never happened in the past year, but did happen prior to that; (c) happened once or a few times over the past year (low frequency); (d) happened once or a few times per month (moderate frequency); (e) happened once or a few times per week (high frequency).
